I declare that stated personal data are accurate, correct and actual and I give my consent to make personal data available to the third parties – companies seeking to fill vacancies. This consent is given for a maximum period of 3 years or until the day of delivery of written withdrawal of this consent by whatever means.
I hereby declare that I was advised about principles relating to processing of personal data and about my rights.
